LONDON, December 7, 2017 – peopleCare Communities and Hogan Pharmacy Partners demonstrated a new communication system for long-term care (LTC) homes that is funded by Ontario’s Health Technologies Fund (HTF).
This project is one of 15 that received funding in the first round of Ontario’s $20-million HTF. The fund is administered by Ontario Centres of Excellence (OCE) on behalf of the Office of the Chief Health Innovation Strategist (OCHIS), and is a part of the OCHIS mandate to strengthen Ontario’s health innovation ecosystem.
“The Health Technologies Fund is already having an impact in the health system because of the collaborations it has created between health service providers, health technology innovators and patients,” says William Charnetski, Ontario’s Chief Health Innovation Strategist. “We are finding new ways to solve our greatest challenges by harnessing the power of innovation to provide better care while creating jobs in Ontario.”
Representatives from OCHIS and OCE were on location for a presentation that included highlights of the Health Innovation Teams’ progress to date.
In partnership with Hogan Pharmacy Partners, peopleCare Inc. is the first in Canada to implement telepharmacy in LTC homes. The innovative system of communication and documentation supports efficient collaboration and decision-making between pharmacist, physician, and nurse. The challenge being solved is the issue of poor communication access being the most frequent cause of medication errors during transitions to LTC homes.
“We believe that pharmacists leading medication reconciliation at times of transition should be the standard in Ontario,” says Jennifer Killing, Vice President, Quality & Innovation of peopleCare Communities. “Through the use of technology solutions, and 24/7 telepharmacy, we have been able to see a reduction in medication errors and omissions, an improved focus on direct resident care due to a release of nursing time and, most importantly, improved resident outcomes in our homes for all residents following their admission or re-admission to our LTC homes"

With a proven track record of delivering programs on behalf of the Ontario government, OCE was selected to deliver the HTF and assist in driving the development of made-in-Ontario healthcare technology while supporting economic growth, co-investing to commercialize innovation and fostering partnerships and collaboration in the health system.

About the Office of the Chief Health Innovation Strategist:
The Office of the Chief Health Innovation Strategist (OCHIS) was created in September of 2015 to fulfill the recommendations made by the Ontario Health Innovation Council (OHIC) established by the Ontario government in 2013. The council submitted is report, The Catalyst, in December 2014.
